The problems in the form are located in only two issues (after 15,000 forms filled by our customers): not being able to press "Continue" after filling the form, and not being able to fill the e-mail field. The curious thing is that I have a PHP page builder application. The page I built is formatted as expected when viewed in IE8 and Firefox but on my works laptop running IE6 the input box is half way down the page rather than the top.</p> <p>I really do think that IE6 doesn't like PHP.
