&#60;p&#62;For checkboxes -- MachForm only sent the selections not the question. Since checkboxes are usually using descriptive labels.&#60;/p&#62;
&#60;p&#62;In your case (a simple yes/no question), I suggest to use Multiple Choice (radio buttons) instead of checkboxes. That way you will get the Question label too in the email.&#60;/p&#62;
&#60;p&#62;Also, using radio buttons prevent your users from choosing two answers all together.

&#60;p&#62;The recipient doesn't know what was the QUESTION at the checkboxes, it only displays the selections, YES or NO.&#60;/p&#62;
&#60;p&#62;hope it makes sense.&#60;/p&#62;
