&#60;p&#62;That's not coming from MachForm. MachForm doesn't prompt for username + password.&#60;br /&#62;
Most likely you have .htaccess file which set your site to prompt for password.&#60;/p&#62;
&#60;p&#62;Try to search into your document root folder and delete the file.&#60;br /&#62;
Or contact your hosting tech support as this is server issue.

&#60;p&#62;I had no problems creating this form and imbedding the code into the site I am working on. However, when I go to test in IE, I get the following pop with a sign on request:&#60;/p&#62;
&#60;p&#62;The server bridalnbeyond.com at bridalnbeyond.com requires a username and password.&#60;/p&#62;
&#60;p&#62;Warning: This server is requesting that your username and password be sent in an insecure manner (basic authentication without a secure connection).&#60;/p&#62;
&#60;p&#62;I double checked the form - it is not password protected. I am able to hit cancel and then fill out the form, however, this would not be acceptable for my clients.
