&#60;p&#62;The quick solution is to add this line below into &#60;strong&#62;config.php&#60;/strong&#62; file:&#60;/p&#62;
&#60;p&#62;&#60;code&#62;$_SERVER[&#38;quot;PHP_SELF&#38;quot;] = $_SERVER[&#38;quot;SCRIPT_NAME&#38;quot;];&#60;/code&#62;&#60;br /&#62;
&#60;code&#62;&#60;/code&#62;&#60;br /&#62;
Better solution is to upgrade PHP :-)

<description>&#60;p&#62;I don’t believe that to be the issue considering I am running other .php apps on our server  with no problems. If you notice when you went to &#60;a href=&#34;http://www.campcondor.org/forms/index.php&#34; rel=&#34;nofollow&#34;&#62;http://www.campcondor.org/forms/index.php&#60;/a&#62; the link shows  &#60;a href=&#34;http://www.campcondor.org/forms/index.php/forms/installer.php&#34; rel=&#34;nofollow&#34;&#62;http://www.campcondor.org/forms/index.php/forms/installer.php&#60;/a&#62; in the address bar it seems to me that something in the software is adding the extra data to the address.
