You see on the sidebar I'm including one of the forms which shows on all pages. I'm using the advanced method of including it into the website but I'm having an issue. Whenever I add a 4th form to the website, no matter which form it is, I get the following error:</p> <p>Fatal error: Cannot redeclare display_machform() (previously declared in /home/myusername/public_html/forms/machform.php:27) in /home/myusername/public_html/forms/machform.php on line 237</p> <p>Can anyone help? </p>
