I found the problem. The form alignment became correct when I removed the "guideline" attached to the "Business Name" field. Can you explain this so I can avoid it in the future?

I do not need to split the form into any columns. I changed the css to make the following three sets of fields to be inline:

Donor's Name and Business Name
Phone and Email and Website
Inline radio buttons on "Do you want the donor to be anonymous?"

But I have removed the inline css for each of these and it doesn't correct the problem. Also, I did insert a small table into the last Section Break, but that is not the cause since the problem still exists if I remove that field.

I'm not quite sure about this one.
Do you need to split the form into 3 columns?

Which fields would you like to have it into a row?

Hi,
My form (url below) was progressing fine until it suddenly shifted several items to only the left third of the space. Some items are spaced normally across the form and some are shifted left. Can you tell me how to correct this?

http://www.unitedwalpole.org/admin/machform/view.php?id=5.com

Thanks for all your help.
