&#60;p&#62;I believe we have had corruption on our database and I don't have a recent backup. Several of my forms are missing and on the ones that still exist, even though the &#34;Edit Form&#34; area looks fine, if you view the form, only a Back and Submit buttons exist. Here's an example: &#60;a href=&#34;http://www.concertonetworks.com/emr/emr-contact-us.php&#34; rel=&#34;nofollow&#34;&#62;http://www.concertonetworks.com/emr/emr-contact-us.php&#60;/a&#62;&#60;/p&#62;
&#60;p&#62;Any suggestions on how I might be able to salvage any of this?&#60;/p&#62;
