On the e-mail settings-send confirmation to email to user, I clicked on more options and made sure "email" was showing in user email address and also that "email" was showing in from e-mail address. I first assumed it was the mail setup and spent time trying to troubleshoot that. After no luck with that, I happened to notice that my form notifications were turned OFF across the board. After going though and turning everyting back on again, all was good (at least so far). </p>
