<description>&#60;p&#62;Searching the forum, I found this; &#34;I fixed it. I went into the ap_forms table and delete the problem form. Then it worked fine.&#34; I did the same thing and it fixed the problem.&#60;br /&#62;
Although a re-install crossed my mind, I'm glad I ran across this solution.

<description>&#60;p&#62;I accidentally deleted table 16 in MySQL, no backup. Now when I try to access Machform I get the following message, select count(*) total_entry from &#60;code&#62;ap_form_16&#60;/code&#62; Query failed: Table 'dbfirstpres.ap_form_16' doesn't exist receive the following message,&#60;/p&#62;
&#60;p&#62;I'm not concerned about the form, I could recreate if needed, but to create more forms I need to access Machform.
