If the machform database already restored you should able to run your machform again with same configuration.

<description>&#60;p&#62;A helpful co worker has just erased our databases! The ones with our WP sites on are easily recoverable with a re-install and a couple of hours work but our machforms are gone!&#60;br /&#62;
we have 8 current active forms that we need to get back up and running ASAP. Any help tips or other would be greatly appreciated. Is there some way we can recover them from what information we have in the machform folder that sits on the server?&#60;/p&#62;
&#60;p&#62;I reinstalled machform with the exact configuration of the previous database but to no avail.  :(
