<description>&#60;p&#62;Yeah, I saw that after I put in this post.  I have used these but in some cases the alignment squeezes fields down to smaller than their original/defined size.&#60;/p&#62;
&#60;p&#62;Speaking of, the forms seem to always size at around 700px wide.  Is this value defined somewhere I am not seeing, so I can change it?

<description>&#60;p&#62;I have the need to create a questionnaire form with multiple fields that are related to each other.  For example:&#60;/p&#62;
&#60;p&#62;3. Age&#60;br /&#62;
	Write the number of family members in each age bracket.&#60;br /&#62;
	____ Birth–5 years	____ 30–39 years&#60;br /&#62;
	____ 6–11 years		____ 40–49 years&#60;br /&#62;
	____ 12–17 years		____ 50–59 years&#60;br /&#62;
	____ 18–29 years		____ 60 or more years&#60;/p&#62;
&#60;p&#62;I love being able to put radio-buttons inline/same line but would like to do the same as multiple-choice but with number fields and text.&#60;/p&#62;
&#60;p&#62;Suggestions?
