<description>&#60;p&#62;Actually mine was as a result of the forms folder not being copied over subsequent to the upgrade, so now mine is working - column_2 and column_3, and the form looks correct, formatting-wise as well.

<description>&#60;p&#62;forgot to mention something that might help determine problem - getting number signs/hash tags in between telephone number fields, and when alerted to fields not properly filled out, the alert is blending right into the fields (it isn't red anymore) - no formatting whatsoever.  Thanks.

<description>&#60;p&#62;Just upgraded and having some issues.  One of which is that after adding the &#34;column_2&#34; text to the CSS area in two fields, they are not aligning.  Other is that my whole form looks funky now.  Have tried switching themes, but the entire alignment and the way the fields are being displayed is really weird.  Not sure what the problem is.  (Have cleared cache.)
