<description>&#60;p&#62;When I am deploying a form using check boxes, only the choices show as column headers in the XLS DB.&#60;br /&#62;
The field label, unlike radio buttons or pulldowns were only one item can be chosen, does not display, which if you had a form using many checkbox fields would be hard to manage on the DB side of things.&#60;br /&#62;
Is there a way to preface the checkbox items with the field label?&#60;br /&#62;
