Check the permissions (CHMOD) on the folder that stores the uploaded files - should be Chmod 755 or 777(?).<br /> the folder would be the "files" folder e.g. within the folder struction like "data/form_X/files"</p> <p>It also mentions file size, so I would also check if you have any file size limit placed (check the upload field properties/options).
