&#60;p&#62;If you just want to apply your own theme that is easy to do but it seems you have already done so.  &#60;/p&#62;
&#60;p&#62;I can see the theme you're currently using (see screenshot).  If that's not the correct theme, you need to apply yours, as per below:&#60;/p&#62;
&#60;p&#62;In the Forms Manager,  click the form you want (should highlight light-blue or other colour depending on the admin theme you use), and then click the button labelled &#34;Theme&#34;.&#60;/p&#62;
&#60;p&#62;A light-box (pop-up) comes up with a drop down box of all the available themes.  Custom themes are listed under &#34;Your Themes&#34; in the drop down box.&#60;/p&#62;
&#60;p&#62;Select the theme you want.&#60;/p&#62;
&#60;p&#62;Click &#34;Save Changes&#34; and then click &#34;View&#34; back in the main form managers list for your particular form - you should see the stand-alone version of the form with the applied theme.&#60;/p&#62;

You can duplicate a form easily.  In the forms manager, click the &#34;Duplicate&#34; button for your form.  It will show up in the list like &#34;[your-form-name] Copy&#34;.   See the documentation for more: &#60;a href=&#34;http://www.appnitro.com/doc-form-manager#duplicate.&#34; rel=&#34;nofollow&#34;&#62;http://www.appnitro.com/doc-form-manager#duplicate.&#60;/a&#62;&#60;/p&#62;

&#60;p&#62;You would then simply and quickly apply your theme to it, but you don't need to copy it, simply follow the first instructions above to change the theme - you can change the original form's theme to the one you want; there's no 'lock-out' once you have applied a theme; you are free to change it whenever you like.&#60;/p&#62;
&#60;p&#62;Note however, if you already have entries in the original form the duplicate will NOT have the entries copied - it merely copies the design and themes etc, but not the existing entries.

&#60;p&#62;But I cannot figure out how to add the theme I created after I finished this form.  Is it to late to change the theme on my form?  If so, How do I copy all the fields I created into a new form that uses my new theme?  Thanks!!!!!
