<description>&#60;p&#62;I'm not sure why (or if?) you created your form using  &#60;strong&#62;&#60;em&#62;phpform&#60;/em&#62;&#60;/strong&#62; (http://www.phpform.org) but if so, it wasn't necessary.&#60;/p&#62;
&#60;p&#62;You should be using Machform to create your forms which also has the full capability to manage those forms, and entries submitted as well as many other features.&#60;/p&#62;
&#60;p&#62;The only thing you need to do to get a form to be in a web page is the code-snippet that is generated when you create and save a form into your web page where you want that form to appear. (The code associated with the specific form is available under the &#34;code&#34; button in the form manager list)&#60;/p&#62;
&#60;p&#62;I would suggest you review the help/support and features links below to better understand how Machform works&#60;/p&#62;

&#60;p&#62;i've generated the PHP form file in the form code section &#38;#38; it has the name booking.php. i've uploaded it into the same directory where machform is installed. when i then try to call &#38;lt;machform-web-url&#38;gt;/booking.php i get an empy page. what could be the reason?&#60;/p&#62;
