@AMurray I did find one flaw with the Matrixx choice that will not work for me. I only have one answer which is Completed and with the option of ""Allow Multiple Answers per Row" it turns that whole section into a required field even if the required box is not checked. I have yet to try it without the "Allow Multiple Answers per Row" check mark NOT checked I will do that now.

You can use check boxes in the Matrix Choice field. In the field properties, check the "Allow Multiple Answers per Row", that changes the rows/columns from radio buttons to check boxes.

Sounds to me like you may use a matrix choice box where you state:
Yes / No
1. I did this X
2. I did this to X
3. I did this three X
4. I did this four X

Where X represents whether they did the suggested activity. Then the form will show all answers. I keep finding that while this form is awesome, we sometimes have to make sacrifices between needs and wants.

Sorry.. I feel for you, I really don't have an answer either - I believe you will need to put in a support ticket as this may involve some coding that I'm not up to par on.

Regards,

I have a form that has multiple sections on it that are check boxes for different actions being preformed. I noticed that on both the review page as well as the Entries that it is only showing those items that were check not the all the items regardless if they were checked or not. Is this something that is possible to request? wow I went from stating an issue to making a request.

For example
1. I did this
2. I did this to
3. I did this three
4. I did this four

If I only check 2 and 4 on the review page I only see 2 and 4 but not the other ones. using the required option is not an option in this case. I hope that all made sense.
