Last night I worked on a form because I was bored that duplicated that form, but without the EXACT look and feel, but containing conditional logic with drop downs for customers to add new names at the adjusted price level. That said, show me a link to the form you produced and maybe I can offer suggestions. This software is for people to build forms. If you are wanting one built for you, there are a number of companies that can do this for you.
