&#60;p&#62;Hence all new forms are using that same incorrect copy.&#60;/p&#62;
&#60;p&#62;Make sure to replace that file with the correct file and try to create new form again.

&#60;p&#62;I have Address, City, State and Zip code.  I have assigned column_4 to all the items Custom CSS Class property.&#60;/p&#62;
&#60;p&#62;Your instructions state: If you are using Left or Right Label Alignment for your fields, this won’t work.  I can't find this setting, I am probably blind - but think I have looked everywhere for it. &#60;/p&#62;
&#60;p&#62;Here is my form, maybe someone can figure out what I am doing wrong.&#60;br /&#62;
