Yuniar,

I contacted the hosting tech support, and you were exactly right. It works now, and I'm very relieved about that!

Thank you very much for the help, and keep up the good work.

Your database server might be up and running, but your website doesn't seem able to connect to your database server.

I suggest to contact your hosting tech support.

Official info from MySQL regarding this error:
http://dev.mysql.com/doc/refman/5.0/en/error-lost-connection.html

Other people having the same problem:
http://blogs.delerue.org/index.php/computing/2007/07/15/mysql_system_error_111

Seems to be mysql server configuration issue.

I tried to log in to my admin page today. I was able to view the log in page, but when I entered my user name and password, I received this error message:

Could not connect: Lost connection to MySQL server at 'reading initial communication packet', system error: 111

This error message is also displayed if I just try to view the form (not from the admin panel on MachForm). I am able to access the database from my web host panel, so I am pretty sure that this error has something to do with the php code.

I'm not sure where to go from here, though. Anyone have any ideas? Thanks in advance for your help.
