<description>&#60;p&#62;The example link is not a Machform form, but  you can split the form's screens/pages in a similar fashion with a Page Break field.&#60;/p&#62;
&#60;p&#62;In the form editor you should see under the fields lists, a &#34;Page Break&#34;  Just drag and drop it into the form and then continue adding fields to the second page.  Then add another page break when you want a third page.   you can addjust the page breaks, drag and drop them to the new positions with your mouse.&#60;/p&#62;

<description>&#60;p&#62;A client has just given me a project that i have to figure out in MachForm. here is the example (http://myvoicemychoices.ca/survey/french/?t=survey). I want to be able to split the form so the first page of the form appears on the front page then when you click continue it goes to the rest of my form. HELP!!!!!!
