<description>&#60;p&#62;Basically, you can use your text editor to create a new blank PHP file.&#60;br /&#62;
So, for example, open Notepad or TextEdit, create a new file and then paste your form code there.&#60;/p&#62;
&#60;p&#62;Save the file and give it a name, something like &#34;myform.php&#34;&#60;br /&#62;
That would do it.

&#60;p&#62;Create your unique filename, a php file.&#60;br /&#62;
Let say &#60;a href=&#34;http://www.mysite.com/myform.php&#34; rel=&#34;nofollow&#34;&#62;http://www.mysite.com/myform.php&#60;/a&#62;&#60;/p&#62;
&#60;p&#62;Then copy the advanced form code there. [Form Manager -&#38;gt; Embed Code -&#38;gt; Advanced Form Code]&#60;br /&#62;
This way you will have unique URL for each of your form.
