Is it possible to create columns that will accept multiple entries... for example, the form will have a section where people can enter the date, the location and a description of their activity several times? The date, location and description would be the headers of three columns. People could access their form and add a new line of info into these columns as needed.

DATE | LOCATION | DESCRIPTION
11/17/08 somewhere we looked over the items
11/19/08 somewhere we made appointment to view more stuff

Hope I am making sense.
