align two text inputs on the same line and form allignment issue

    I need help with arranging the text fields as described in this post, i followed the instructions but now its all showing not in a order, also if there is error highlight only one of the fields work the other one don't.

    here is my current form:

    I am working on a deadline and i need my form to look like this:

    also would like to know how do I change the submit button to an image as a button
