   A number of my users are getting the following crash. I would say that about 1 in 15 users get this error. What does it mean and what should I do about it?

   SQL: [80] INSERT INTO ap_form_10951() SELECT from ap_form_10951_review WHERE id=? Params: 1 Key: Position #0: paramno=0 name=[0] "" is_param=1 param_type=2 Query Failed: SQLSTATE[42S22]: Column not found: 1054 Unknown column '' in 'field list'

   The problem mostly coming when you edit the form and meanwhile someone submitting the form.
   We suggest to disable the form when edit the form, so it can not be accessible when you make the changes.

   This has happen again. No editing was underway when the error reappeared. Could there be another source of this error. In both cases, no editing of the form was underway.
