does this form support inserting image?

    can this form support image to be integrated directly into the form itself?

    You can add a "Section Break" and insert your image tag there.

    Add a section break and insert your IMG tag to the "Field Label" input box.

    yes, you can use CSS code to make it centered. Paste me the URL to your form and I'll give you the code.
