   All forms are working and showing up in database, but when the forms are submitted the end user gets an error 500 on the browser and we do not receive the email.

   Update, when I change "Use SMTP Server to Send Emails" to unchecked the form processed properly. when I have it checked is when I get the error

   Probably that is because you hadn't filled in the SMTP details inthe panel headed "Use SMTP Server to Send Emails".

   However Machform can make use of the internal mechanism "sendmail" or "phpmail" and doesn't need an extermal mail server. All you need is to specify the email address to receive the incoming submission notifications.

   If you use SMTP you need to specify

   mail server
   Port Number
   Username
   Password
   etc.

   Fill in the details for your mail server in the panel, "Use SMTP Server to Send Emails".

   You will find these clicking "More Options" in the "Send Notifications to my Inbox" panel.
