   I keep get the following error when attempting to create a new form:

   Warning: require(includes/JSON.php) [function.require]: failed to open stream: No such file or directory in /home/timmer/public_html/machform2/edit_form.php on line 16

   Warning: require(includes/JSON.php) [function.require]: failed to open stream: No such file or directory in /home/timmer/public_html/machform2/edit_form.php on line 16

   Fatal error: require() [function.require]: Failed opening required 'includes/JSON.php' (include_path='.:/usr/lib/php:/usr/local/lib/php') in /home/timmer/public_html/machform2/edit_form.php on line 16

   Any suggestions?

   From the error message, it seems "JSON.php" file was missing from your machform "includes" folder. Try to re-upload "JSON.php" file to your "includes" folder.
