   I have been creating forms with no problem for a long time now every time I generate the form
   it shows the form name twice like this:

   Email Contact form:

   Email Contact form:

   name
   email
   phone
   comments

   I can delete the form name altogether but that's not really a solution. It shows up this way on webpages and in the application when you select
   view. The first one is larger than the other for some reason also.

   Anyone seen this or have a solution?

   Do you mean it comes up something like below - if so it probably means you have used the same text in the "Description" as in the "Title" in the Form Properties of the form editor.

   Please can you provide a link for your form(s) showing this behaviour?

   That happens when I edit the header, but not when it's displayed to the user as far as I know. I assumed it's becuase the header has a table and a graphic in the table.
