Form Won't Load (blank page)

    One of our forms just recently stopped loading. Instead it's just a blank white page, no error messages. We have it iframed into the site and it used to work fine. Haven't made any changes to machform or the form itself except to add an autoresponder.
    All our other forms are fine: it's just this specific one.

    Ok well I know it's the CSS, but I'm confused because it wasn't changed since the time it was working.

    Not sure what went wrong but I just tweaked the styles from a working form and uploaded it.
