Forms not showing up at all

Our sites are self-hosted on an IIS server running PHP 5.4. I'm able to use MachForm just fine if I use the Popup method...but nothing shows up if I try and embed using Javascript or the PHP embed code. If I put the same page over on my Godaddy server, it works just fine. This indicates to me that the issue resides with our server (setting, perhaps?). You can see an example here: <http://georgiamri.com/about-us/contact2/> Under the paragraph of text is our form, embedded using Javascript. (you can see the form by clicking the Email button at the bottom of the page...and then clicking the button on the new page). Any ideas on what I need to change would be appreciated.

Check your security settings, make sure that the permissions are set correctly.

Thanks Saghalie.
According to our SysAdmin (corporate policy - only they can adjust permissions), "everyone has full control to the data folder." I guess for now, I'll just leave the forms as popup windows (which works just fine, too).
