   is there any documentation with this software?

   Also installing it on godaddy hosting and using MySQL 5.0 did not work swirched to 4.1 and it took it. Why not 5.0?

   How do I get the Control Panel up?????
   I created a form, how do I get it into my webpage ??? (copy and paste???)

   appreciate some assistance
