   How does this software really work?

   I am a little confused. I need to create free standing forms as well as integrated forms into websites but do not have enough technical knowledge to get that done.

   Doe MachForm create the code and the database that can be installed on the server or does it store it on MachForm Servers? So, to make things clear, does this work like if I went to Dreamweaver, created the form fields and then coded all the PHP and them uploaded that to my server, or does this work another way.

   Machform is a install-yourself product not a hosted product

   Machform is a product you install on your own hosting service. On purchase, you receive all PHP and other files, have access to the source code. You don't need Dreamweaver or any web editor to edit anything except perhaps the config.php file to enter the database details and change a few options. (recommended that you use Notepad++ or similar text editor rather than something like Dreamweaver, Expression Web etc).

   You will need to create the MySQL database and database user, (easily done through Plesk, cPanel or PHPMyAdmin etc).

   You install the Machform product on one domain/server only (as 1 only is allowed for the basic $40 licence). From there you can create multiple forms, and integrate into any other web site by means of a snippet of code (PHP snippet for advanced users, or HTML snippet (iFrame) for basic).

   Freestanding Forms
   You can't create freestanding forms with Machform product itself, however there is a lite version available (with no processing capability, just provides the form itself): http://www.phpform.org

   This one you can open in Dreamweaver, add your own processing script and upload to your website's server, but it doesn't have the comprehensive features of the full Machform product.

   So, I install on my server, generate the form just like the demo, get the code from that form and edit it for what i need it to do, then place that code in the directory it needs to be in.

   Basically, do I have complete control over the code in the end so that I can edit it to send to SQL or to email, etc

   Not exactly. So MachForm won't generate separate file for each of your form. Instead, an embed code will be provided. You can use the embed code to put the form to any page you want.
