    Add Check box field with CHECK BOX and field where user fills in additional items. thank you

    You can go to the field property of your checkbox and make sure to enable "Allow Client to Add Other Choice".

    Yes, that seems to add one choice, one check box with one blank text field. My client wants 5-6 check boxes each with a with blank text field for user to add items.

    Ah.. I see. I'm afraid it won't be possible to do this. Only one "other" text field is available for one checkbox field.
