How to create custom header?

I have no idea how to do this? Is there a forum topic or support topic that address this?

Can you elaborate on what you're trying to do? "Create Custom Header" - what exactly do you mean by this?

I would also like the same to do the same thing. I want the form to look like I created it myself I do not want my competition to know what I used to create my forms. i don't want a powered by machforms I want it powered by my domain name or none at all.
I do not want the machform logo any where on my site.. I want my own logos instead so how do i remove it.. I was thinking to replace it on my server with my own logoPosted 16 years ago # -
Yup. I've got to agree with the sentiment expressed in this thread. Forms should be branded with the form name, at the top of the page, and "Powered by MachForm" at the bottom should be optional through the GUI.
I understand that your guys get crazy good pagerank from all these links, and if you weren't charging for your software, this would be okay.

Also, the image at the top of forms and success pages is just images/machform.gif
