   hi
   i'm looking for a tool to easily customize print forms from a mysql database.
   Machform is perhaps the right tool, but I did not found how to have multi column
   forms, as each new panel is set under the previous one.

   also is it possible to include a logo in a panel ?

   You can implement some CSS adjustment to allow multiple columns within a form.

   Regarding the logo, do you mean you would like to insert the logo into your form?
   If so, yes this is possible, simply edit the logo section within the form CSS file.
