New field if "Yes" or "No" option chosen

    I have a radio button field with a Yes or No question. If the user checks "Yes", I would like a new field to be populated bleow this asking for addt'l info. How do I accomplish this?

    Is there a way to possibly modify the code in any manner to accomplish this?

    Sorry, it would be too complex to modify the current code to achieve this.
    If anybody ever done a modification to do this, please post it here.
