I cannot access my admin login page, instead I get a page saying 'The page cannot be found'.
Any suggestions please.

Did you publish/transfer all the required files?
Give the error says "The page could not be found", this indicates 1) the file is not on the server at all or 2) it's not in the place it is expected to be.
Make sure you've FTP'd all the files to the correct folders.
The login screen, under normal operating circumstances should come up immediately if you browse to the (default) URL such as "www.yourdomain.com/machform/"
If you have installed to a different folder etc, make sure you're entering the correct URL pointing to the folder containing the login script file(s).
That's all I can suggest because I don't know why the "file cannot be found" would come up for any other reason other than the file(s) not being on the server in the expected locations the program is looking for them in.

Is the file index.php exist within your machform folder? If not, try to reupload that file from the original package.

I'm having the same issue. Did you resolve this? I am 99.9% sure I'm doing everything correctly. I'm pretty familiar with this kind of stuff, but clearly something isn't working...

I'm getting an "internal server error" and it asks me to notify the webmaster or check my server error log for more details.
When I check the error log, this is what I find:
[Fri Jul 1 22:11:41 2011] [error] [client 50.54.252.86] Premature end of script headers: [edited out web address]forms/installer.php
SoftException in Application.cpp:601: Directory "[edited out web address]/forms" is writeable by groupAlso, I can access folders and images by typing in the same http path, so I don't think I am doing that wrong.

this seems to be folder permission issue. Try to change the permission of your "forms" folder and all files under that folder to 755.
The only exception is the "data" folder, this one (only this directory) might need 777 permission. If the problem persist, please contact us directly and we'll help you.

I've done what you've asked, and for some reason it is still not working. I can find files within the folder I've created (images, etc) but the login page won't load. How do I contact you directly?
