Not sure what broke between my host and clients host

    Hello,

    I have just completed a form that looked perfect on my server:
    http://forms.imokon.com/view.php?id=10

    and upon uploading it to my client's server

    http://habibtispa.com/forms/form.php

    it seems broken, it never did this before and I'm not sure what the problem is. I am using the basic html code and not the php. I don't think the height had to do with it, and some of the CSS looks ok but the rest makes it look like it's missing.

    I tried .html and .php extensions, I tried adding the html, body, tags and without...

    What could the problem be?

    And the funny thing is we share the same hosting provider.
