Would it work for you to add your logo in the confirmation email rather than just on the form? For example fill out the form at http://site.beautifulsaviour.org/forms/view.php?id=32 and you will get a .copy of our church's logo in the first line of your confirmation email. That's done by putting code like this into the Confirmation Email Content field:
<img src="http://site.beautifulsaviour.info/forms/WebColorLogoNoWords.gif"> {entry_data}
Obviously you have to tailor the image name and location to your logo. Does that help?

Oh, here we go again with the backticks.

Thanks DaveVoss.
That will do temporarily. It is a great option.
But we also need to have the form with the logo when printing it out, so if we choose to print the form directly from the admin panel, it'll be great if it prints out the logo too. But i guess, that we can also print the email, and then we have the logo there.

OK, then to print your logo when you print the form,
1) in the Forms Manager select "Edit Themes".
2) In the drop down box select the theme for your form.
3) Click on the word "Logo",
4) Click on the "A" for the Logo and
5) select "None".
6) Save the theme.
7) Then edit the form and in the Title Field or Description field for the Title, add your logo using the code for an img above as a template for your logo. Again you can go to the form address I gave you above and see how it works.Now redityo, yuniar, or others may have other solutions for you, but for now that works.

The form logo is using a background image. If you need to print it as well, make sure to select the option to "print background images" on your browser print preference. This option usually turned off by default, so you need to enable it.
