First it says MachForm when email is received how do i change that?

Second: How do i clean up the email i have a huge form when i open it its just confusing?

Third: How can i have two fields beside each other? My form is very long and i need to shorten it.

ok maybe i didnt i edited

define('NOTIFICATION_MAIL_FROM_NAME',''); //default is 'MachForm'

define('HostApply',''); //default is 'MachForm'

and wheni received the email it says NOTIFICATION_MAIL_FROM_NAME

Please refer to this post for a sample regarding the email customization.

What do you mean by cleaning up the email? Do you want to remove some of the fields from the email?

Regarding the third question, I'll get back to you shortly on the other thread.

Hmm..your form is quite long indeed.

What you can do at this moment is to add a break in between.
Let say you want to add a break for every 5 fields.

Open this file: includes/helper-functions.php
Find this code (should be around line 370-375):
```
foreach ($email_data as $data){
	$email_body .= "{$i}) {$data['label']} {$data['value']} ";
	$i++;
}
```
Replace that code with this one:
```
foreach ($email_data as $data){
	$email_body .= "{$i}) {$data['label']} {$data['value']} ";
	$i++;

	if(($i % 5) == 0){
		$email_body .= "------------------- ";
	}
}
```
That code should add a break for every 5 fields.

Sorry, try to change this line:

to this:
