Secure Form Error

The page is loaded under SSL, and the form itself is loaded with the https URL. The form data is submitted, but the page still shows the form as if the submission was not successful.
The debug console in Chrome shows the following:
Mixed Content: The page at 'https://myform.example.com/form.html' was loaded over HTTPS, but requested an insecure form action 'http://myform.example.com/embed.php?id=80978&done=1#main_body'. This request has been blocked; the content must be served over HTTPS.
How do I set the form action to go to https instead? Thank you.
