I don't know what mail option you're using PHP sendmail or SMTP?
Sendmail should automatically work providing your host has "sendmail" PHP settings on (or perhaps you can turn it on if you have access to the PHP.ini file (?).
If Sendmail doesn't work (it might be something your host doesn't allow or support), try setting the SMTP mail server setting. This is the standard out-going mail server you would set up for your email in a client like Outlook.
Go to Settings, Check the box "Use SMTP Server to Send Emails", then fill in the information. Anything you don't know, ask your host for the settings, but the email address is the one you want to receive the email at, the username and password is that for that particular email box. The Port Number may vary from host to host - it may not be the default "25". The mail server would be something like smtp.yourdomain.com - again this varies according to your host.
If that doesn't work, I'm not sure what else to suggest.
If you lodge a support ticket to Appnitro, with your Machform detail and FTP detail, they can probably check your installation of Machform.
