test hyperlink not working through email??

    Hello,
    I am trying to create an email using a text hyperlink for my client to just click on to fill out a questionnaire. I used the simple link... But it is not working. Is there a URL that I can us to do this?

    I just created simple form to be used for email . select the simple link from the cod , used on email it works fine
