Unable to submit the form

I am unable to submit the form. I am able to succeeded the installation of Machform in my server.
The following error message i am getting it, details are given below.
I recently purchased your project, and successfully able to install in my portal. After once i created my forms, i am unable to get the feedback and auto replay to the customer. I already tried using gmail smtp and also with my private email account.

Previously i got some error once submitting (i enable the review option before submitting for that form) as ID Required (Chrome Browser), After once i disabled the option i am getting the following error message which is given below. And for your kind information, I already provided full permission to all directories including the root folder - machform.
And, I am getting some error as well once submit the form

There is a problem with the resource you are looking for, and it cannot be displayed.Thanks in advance, please kindly update me on the same ASAP., Waiting for your replay Sir.
Its bit very urgent... If you need any further info, please kindly update me on the same.

Some times if we enable the review before sending, it shows message as - ID Required.

Hey there!
An internal server error is an error on the web server that you’re trying to access. That server is misconfigured in some way that prevents it from responding properly to what you’re asking it to do.
In other words, you need to check the permissions on your site... the best way to do this is to look at your access logs and your error logs - the ones for your web server.
If you can't figure it out, please fill out a support request and maybe the team here can access your web server and find out what is mis-configured with regards to machform.

Thanks a lot for tips & ticks, let me check my log and will get back... Seems, i am not getting any errors not, i just gave CHMOD777 full permissions to all my MachForm folders and sub folders.
