URL Parameters

What is the format that I need to use to pass URL parameters to an embedded form? What I saw is that you can do it using the view.php, but I am trying to use the form embedded on our main site. I am guessing that I need to use an actual PHP form file, not an embed code, but is there something special that I need to do to get the parameters to pass through from the url?
Sorry if that is confusing.
EDIT: I tried to embed a PHP form file, or use the PHP embed code, and neither would display anything on the page. I am using IIS, and the form installation is on the same server as the site.

Are you embedding the form into a CMS? If so, your CMS most likely filtered the URL parameters.
Or you are simply using a plain PHP page without any CMS?MachForm Founder

It is a plain PHP page without CMS. The URL is contronex.com/authorization. I want to embed that form with prefilled information that we include from quotes.
