Creating same form mutiple users

    Hello,
    I have one form that I need to use for 30 to 40 people. Its a sales form. I would like to be able to ID the form when it comes in to the sales person, without having to create 40 Mach Forms in the system and the only varible is a person.

    Is there some way in php or HTML to add the sales person name and it get submitted to me with the form?

    I would also like to do something like this.

    Maybe if I could just add a hidden form field which sets the email address to be delivered to.

    I have found a solution. Looking at your post again machview I think we are looking for different solutions. Anyway I hacked together my solution reading this forum posts.
